Coast Plaza Hospital Wound Care Program is designed to address the entire scope of issues brought on by wounds that refuse to respond to conventional treatment. Our specialized program includes:
• Treatment recommendations based on the latest advances in care and AHCPR guidelines.
• In-home nurse consultations.
• Coordination of patient care.
• Nurse assistance with all pre-op preparations, including scheduling, transportation and in-hospital course.
• Communication coordination between the attending physician, surgeon, patient and family.
• Reports to attending and referring physicians.
• Weekly nurse visits following surgery until wound heals completely.
To better address a patient’s needs, team members are prepared to perform evaluations in Skilled Nursing Facilities, at home, and in hospitals. When necessary, they arrange for patient transportation to Coast Plaza Hospital for treatment, and continue with follow-up examinations after the patient is released.
